Photograph Guidelines:

One Original Passport Size Photograph (Colour Only) is required. Scanned or photo copies are not acceptable. The Photo should be inserted into the pocket on the front page of the application form. (Photos should not be left loose in envelopes as they may become lost).

The photograph submitted with your application must comply with the following:

  • Be not more than 6 months old and in colour

  • Be at least 45mm in height and 35mm in width

  • Should be a full front view of the licensee’s head

  • The picture should be sharp and clear and taken against a white or light grey background

  • Sunglasses, tinted glasses or any other eyewear which obscure a person’s eyes are not acceptable

  • Only head coverings worn for religious reasons are permitted, no hair bands or other adornments

  • All of your facial features from bottom of chin to top of forehead must be clearly shown

Applications with photographs which do not meet the above requirements will be rejected and returned to the applicant
